SWAN Primary Care Network (PCN) is a group of local GP practices and healthcare providers working together to deliver coordinated, community-based services tailored to their patients’ needs.

SWAN Primary Care Network serves a population of around 35,000 people across the South Wigan Ashton North area, bringing together seven practices: Kumar Family Practice, South Wigan Medical Practice, Marus Bridge Practice, The Grange Practice, Winstanley Medical Centre, Bryn Cross Surgery, and Medicentre.

As an extension of these practices, SWAN PCN works collaboratively with a wide range of health and wellbeing professionals to improve access to care, share resources, and deliver services that address local health priorities.
